Students are required to review the following information on the University Portal prior to Enrolling or Waiving the Student Health Insurance plan:

For Students Waiving the plan
The waiver information you supply is for one term only. Please be advised that any student providing false or misleading information during the waiver process will be considered in violation of the SGU Student Code of Conduct.
What you will need to waive the plan:
  • Health insurance provider name
  • Policy number
  • Health Insurance provider address and phone number
  • Front and Back of Personal Health Insurance Coverage Car
  • Document from Personal Health Insurance, on company letter head, stating the duration of your policy and that it’s currently active
    • For students studying in Grenada: Passport information
    • For dependent coverage: If you are choosing to purchase the dental or vision plan for your dependents, please make sure you have the following information to process their enrollment:
      • Dependent name and date of birth
      • Dependent Country of citizenship
For Students Enrolling in the Student Health Insurance Plan
SGU students enrolled in the SGU Student Health Insurance plan, whether studying in Grenada or during clinical rotations in the US and/or other locations, will be covered under the scope of the plan regardless of location and country of citizenship.
What you will need to enroll into the plan:
  • Beneficiary contact information (for the life insurance portion of the coverage)
  • For students studying in Grenada: Passport information
  • For dependent coverage - If you are choosing to purchase any of the plans for your dependents, please make sure you have the following information to process their enrollment:
    • Dependent name and date of birth
    • Dependent Country of citizenship

*Social Security number - In accordance with the US HealthCare Reform Laws, Health Insurance Providers are required by law to obtain sensitive information such as your social security number for reporting purposes to the Internal Revenue Service.
