prior to enrolling or waiving

For Students waiving the plan

In order to complete the waiver you are first REQUIRED to read through the SGU web page entitled 'For Students Who Waive the Plan' in the Student Health Insurance section of the University Portal. The waiver information you supply is for one term only. Please be advised that any student providing false or misleading information during the waiver process will be considered in violation of the SGU Student Code of Conduct.

• To complete the waiver you will need your health insurance provider name, policy number, provider claim address, and provider phone number.
• For students studying in Grenada: Your passport information
• For dependent coverage - If you are choosing to purchase the dental or vision plan for your dependents, please make sure you have the following information to process their enrollment:
  • Dependent name and date of birth
  • Dependent Country of citizenship

For Students Enrolling in the Medical Plan

SGU students enrolled in the SGU sponsored plan, whether attending class in Grenada or in clinical rotations in the US and/or other locations, will be covered under the scope of the plan regardless of location and country of citizenship.

In order to enroll, you will need:

• Beneficiary contact information (for the life insurance portion of the coverage)
• For students studying in Grenada: Your passport information
• For dependent coverage - If you are choosing to purchase any of the plans for your dependents, please make sure you have the following information to process their enrollment:
  • Dependent name and date of birth
  • Dependent Country of citizenship

Please note: *Social Security number - In accordance with the US HealthCare Reform Laws, Health Insurance Providers are required by law obtain sensitive information such as your social security number for reporting purposes to the Internal Revenue Service. This will prevent you from incurring a penalty. †"Individual mandate" when filing your federal income tax return. († The penalty fee you'll pay on your federal income tax return)
